I stopped riding my touring bike a few months ago because I stripped the nut that the clamp bolt used. Flash forward to yesterday, replaced the bolt, new chain and cables, etc. Now I can't get my front derailleur to shift to my outer ring. I have the height adjusted correctly and I've played with multiple angles of approach, including the correct one. Anyone know what could cause this? The derailleur+ring combo have performed fine in the past, so I don't know why I can't coax them into working properly.
Specs: Tiagra triple, clamp-on, bottom-pull. Salsa unramped rings
TLDR: Front derailleur won't go out to outer ring. Why?

can you shift to the outer ring by manually pulling the cable (where it runs along the downtube)?
You adjusted the low and high limit screws correctly?
I know how to adjust limit screws after the fact, but don't know where to set them when replacing cables. Couldn't find this info in my internet searches.

Yeah, no problem there. Well, I think my bar-end isn't holding tension the way it should, but that's a different issue.

actually, that might be precisely the issue. If you can manually shift by tugging the cable, then the issue is probably with the shifter, not the derailleur. (or maybe not enough overall tension in the cable) try resetting the cable tension, also try tightening the bolt that holds the barcon together (i.e. the one that is basically the axis along which the barcon pivots)
it also sounds like the limit screw isn't the problem if you can shift by hand, but just fyi, I do this: back off the limit screw (your not going to fuck things up if you jump the chain off the cogs while the bike's in your stand). tighten the limit screw a 1/4 turn at a time until the shift the shift to the big ring starts to get tough. then back off a 1/4 turn, see if it shifts better. if not, repeat.
I would completely remove the FD and clean it up to make sure there isn't any gunk interfering with the limit screws or a crack at the inner chain cage.
Do the same with bar end shifter, see if you can get it to hold tension again. That is a critical issue regardless.
Once everything is verified Follow the procedure linked above for a brand new FD installation. Getting the angle and height can be annoying but stick with it.
Another good procedure reference is the original Shimano datasheet which should be saved online as a PDF if you type in the Shimano part number FD-????.

IIRC, for effective adjustment of both the upper and lower limits of the front DR, follow these guidelines.
When re-attaching the FD cable, make sure that the FD shifter is already clicked all the way to the lowest gear setting. Most likely, if you are unable to shift up to your upper wheel, it's because there's another click you need to make on the downshift to lower the shifter to its lowest setting.
Once you're at the lowest shift setting, attach the cable so that it's relatively tight. Adjust lower limit screw at this time to ensure your shifter is at the optimal low limit setting.
Then, shift it up all the way with the shifter, and then adjust the upper limit setting appropriately.
