I have a bike that could use a new fork with a 1" steerer and, if possible, double eyelets. Does anyone have a suggestion for where to pick one up? Surly only makes a 1 1/8" in the style I want. Would Rivendell be a reasonable (i.e., affordable) solution? Any local options?

What bike is this for?
Sounds like you want to run a front rack, do you have it, not all racks use the upper of double eyelets.
you may need to find a used fork, i've never heard of rivendell selling stock forks alone, like from an atlantis, custom forks will be pricey.
Yeah, now that you mention it, the db eyelets are kind've superfluous. Thanks for the link. The tange prestige looks nice if I decide to go threadless, but otherwise the other fork looks like it'll work just fine.

How long of a steerer do you need? How much of it needs to be threaded? What fork offset do you want? What kind of tire/fender clearance?
I might have something, and if not I have some more forks coming soon.

What brake style?
I too have one if you pick the right size, brake type, and like blue.

I'm not sure off the top of my head, but I think I need a steerer somewhere in the neighborhood of 17cm. The fork should have an offset of 5cm or just slightly more. Tire/fender clearance should be good, enough for 32s with fenders at least. Even though it's a touring frame it was made for centerpull caliper brakes, so no canti posts. However, I don't care if the replacement fork has canti posts--so much the better, in fact.

fork off!
Most shops have a buch of forks in the back room. I know every shop, including my present place of employment, has them. Ride your bike and stop by.
