pmwiki fits the bill. php, but no mysql- just flat file storage. page-granular password auth by default (i.e. kind of like the last one, but can set different passwords for each page if you want), but there's a UserAuth cookbook recipe to plug into mysql.

also it has documentation that isn't written in french.
