I think when you pull the chain, the drum touches your tire and spins. Sirens work like a centrifugal pump, sucking air in and accelerating outwards, then the moving "ports" on the drum let it out in bursts, which creates the pressure pulses we hear as "OMG IT'S THE END OF THE WORLD" sounds.

I had one of thes on my old Schwinn until it ripped appart and the housing got run over by a car. I still have it at the shop. I am going to bid on one on ebay and put it on my bike. They are so cool!
